Job Summary

To improve members' health and wellness through tele-pharmacy, the full-time Telehealth RN Case Manager will provide telehealth coaching, develop clinical outreach programs, and support pharmacy staff while working remotely.

Key responsibilities
  • Develop and update clinical algorithms, pathways, and call scripts to enhance service delivery
  • Provide clinical outreach to members and prescribers to improve care coordination and health outcomes
  • Collaborate with department staff to maintain high-quality clinical programs and ensure compliance with regulations
Required qualifications
  • Associate's degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required; BSN preferred
  • Current, active Registered Nurse (RN) license in good standing with the applicable State Board of Nursing
  • 2+ years of clinical experience, including direct consultation experience
  • Experience in a clinical call center and as part of a multi-disciplinary team is preferred
  • Knowledge of healthcare industry practices and HIPAA regulations
