Job Summary

To support members in navigating their healthcare needs, the part-time Telehealth Triage RN will perform clinical triage, care planning, and education remotely, acting as a trusted advisor to improve clinical outcomes and reduce unnecessary healthcare costs.

Key responsibilities

• Conduct clinical triage to assess patient health concerns and direct care using evidence-based protocols
• Provide clinical navigation support, including condition education, care options, and compliance assistance
• Document nursing assessments and member responses accurately while advocating for members throughout their healthcare journey

Required qualifications

• RN, ADN, or BSN degree
• Active RN license in a compact state
• 5+ years of clinical experience, preferably in family medicine
• Triage nursing experience desired
• Care management or chronic condition support experience preferred
