Telemedicine Nurse Practitioner

Job Summary

A company is looking for a Telemedicine Nurse Practitioner/Physician Assistant.

Key Responsibilities

• Perform telemedicine visits to assess stroke survivors for rehabilitation device eligibility and write prescriptions when appropriate
• Determine appropriateness for Principal Illness Navigation (PIN) services and develop disease-specific care plans as needed
• Refer patients to internal and external supportive services and prepare for visits by reviewing chart notes and completing documentation

Required Qualifications

• Experience working with stroke survivors is a plus
• Bilingual abilities are a plus
• Multiple active state licenses preferred, especially in Texas, California, or Illinois
• 3+ years of experience in a neurology or physical medicine setting preferred
• Must be a licensed Physician Assistant or Nurse Practitioner
