Telemedicine Physician Partner

Job Summary

A company is looking for a Physician Partner to provide telehealth services and support community health initiatives.

Key Responsibilities

• Conduct telehealth visits, prescribe medications, and provide clinical guidance to patients
• Collaborate with Community Health Workers (CHWs) to assess patient needs and eligibility for services
• Maintain patient records and provide clinical consultation to CHWs for patient support

Required Qualifications

• Active, unrestricted MD or DO license (NPs licenses also welcome) and current residence in the United States
• Minimum of 2 years of experience in family medicine, internal medicine, geriatrics, or preventative medicine
• Commitment to health equity and addressing social determinants of health
• Ability to operate effectively as an independent provider with strong diagnostic skills
• Prior telehealth experience preferred and proficiency in using electronic medical records and virtual communication tools
