Telemedicine Physician Partner

Job Summary

A company is looking for a Physician Partner to provide telehealth services and collaborate with Community Health Workers.

Key Responsibilities

• Conduct telehealth visits with patients, including consults and medication prescriptions
• Collaborate with Community Health Workers to assess patient needs and eligibility for services
• Provide clinical consultation and maintain accurate patient records

Required Qualifications

• Active, unrestricted MD or DO license; NP licenses also accepted
• Minimum of 2 years of experience in primary care settings
• Commitment to health equity and addressing social determinants of health
• Experience in providing clinical support to allied health professionals
• Proficiency in telemedicine and navigating digital tools
