Telemedicine Primary Care Provider

Job Summary

A company is looking for a Telemedicine Primary Care Provider.

Key Responsibilities:

• Treating patients remotely over a telemedicine platform in a team-based model of care
• Managing an adult patient panel, attending to chronic, acute, preventive, and palliative care needs
• Working with a team of nurses in the field who visit patients and practicing self-management skills training

Required Qualifications:

• Active California License and licensed Board of Directors or Eligible Board in Internal Medicine, Family Medicine, or Geriatrics
• At least 3 years of clinical practice and recent relevant experience
• Experience with diagnosing, assessing, and treating patients with chronic conditions
• Experience with maintaining a patient panel and working with nurses in the care process
• Comfortable with technology, including EHR documentation and videoconferencing/telemedicine
