A company is looking for a Social Services Care Manager to work remotely nationwide.

Key Responsibilities:

Assess and evaluate members' needs and requirements to achieve optimal wellness state
Guide members/families towards appropriate resources for care and wellbeing
Complete required assessments, create care plans, and update member documentation

Required Qualifications:

Master's degree in Social Work from an accredited university
Licensure in Master's degree field of study
Passing of Master, Advanced Generalist, or Clinical ASWB Exam
Minimum 3 years of experience as a licensed Social Worker
Ability to manage multiple priorities and use multiple computer applications simultaneously
