Key Responsibilities:

• Assess and evaluate members' needs to guide them towards optimal wellness
• Facilitate interaction with resources for member care and wellbeing
• Create and update member care plans and complete accurate documentation

Required Qualifications:

• Master's degree in Social Work from an accredited university
• Current, unincumbered, social work license: LMSW, LCSW, LICSW
• Minimum 3 years experience working as a social worker in a healthcare setting
• Must have passed Master, Advanced Generalist, or Clinical ASWB Exam
• Proficient in Microsoft applications including Word, Outlook, Excel
