Job Summary
To support the Commercial Products Care Management team, the full-time Telephonic Care Manager RN will coordinate care and provide health education to Health Plan members through telephonic collaboration while working remotely during standard daylight hours, with occasional evenings and weekends required.
Key responsibilities:
- Conduct comprehensive assessments of members' medical, behavioral, and social needs to develop individualized care plans
- Engage members in preventive health care services and assist in scheduling necessary screenings or diagnostic tests
- Document all interactions and care management activities in the Health Plan's tracking system to identify trends and opportunities for improvement
Required qualifications:
- Minimum of 2 years of experience in a clinical setting and case management nursing
- BSN preferred; home health and oncology nursing experience is a plus
- Registered Nurse (RN) licensure required
- Case management certification or approved clinical certification preferred
- Ability to interact professionally with healthcare professionals and proficiency in computer use
