Job Summary

Key Responsibilities
- Completes telephonic outreach to members and assists with scheduling provider appointments
- Educates members on care gaps and connects them with community resources
- Maintains schedule adherence and ensures compliance with health information regulations
Required Qualifications
- High School Diploma / GED or equivalent work experience
- 1+ years of call center or telephonic customer service experience
- 1+ years of professional experience in an office setting utilizing telephone and computer
- 1+ years of healthcare, insurance, or community outreach experience
- Ability to work full time, Monday - Friday, with flexibility for shift schedules
