Job Summary
A company is looking for a Telephonic Care Navigator to provide support and resources to members remotely.
Key Responsibilities
- Conducts telephonic outreach to members and educates them on care gaps
- Assists members with social determinants of health and connects them to community resources
- Maintains confidentiality of health information and meets performance metrics
Required Qualifications
- High School Diploma / GED or equivalent work experience
- Must be 18 years of age or older
- 1+ years of call center or telephonic customer service experience
- 1+ years of experience in an office setting using telephone and computer
- 1+ years of experience in Healthcare, insurance, or community outreach
