Job Summary

A company is looking for a Telephonic Care Navigator to provide support and resources to members remotely.

Key Responsibilities
  • Conducts telephonic outreach to members and educates them on care gaps
  • Assists members with social determinants of health and connects them to community resources
  • Maintains confidentiality of health information and meets performance metrics
Required Qualifications
  • High School Diploma / GED or equivalent work experience
  • Must be 18 years of age or older
  • 1+ years of call center or telephonic customer service experience
  • 1+ years of experience in an office setting using telephone and computer
  • 1+ years of experience in Healthcare, insurance, or community outreach
