Telephonic Care Navigator

Job Summary

A company is looking for a Telephonic Care Navigator - National Remote.

Key Responsibilities:

• Conduct telephonic outreach to members for post ER visit assessments
• Complete telephonic assessments with members
• Facilitate follow-up for members and perform other assigned duties

Required Qualifications:

• High School Diploma / GED or equivalent work experience
• Must be 18 years of age or older
• 2+ years of experience in a physician's office, SNF, ALF, or outpatient clinic
• Experience working in cross-functional, inter- and multi-disciplinary teams
• Proficiency with EMRs and knowledge of medical terminology
