Job Summary

A company is looking for a Telephonic Care Navigator.

Key Responsibilities

• Conduct telephonic outreach to members for post ER visit assessments
• Complete telephonic assessments with members
• Facilitate follow-up for members and perform other assigned duties

Required Qualifications

• High School Diploma / GED or equivalent work experience
• Must be 18 years of age or older
• 2+ years of experience in a physician's office or outpatient clinic
• 2+ years of experience with outbound calls to patients/members
• Experience working with EMRs and knowledge of medical terminology
