Job Summary

A company is looking for a Telephonic Care Navigator.

Key Responsibilities
  • Completes telephonic outreach to DSNP/CSNP members using various outreach methods
  • Educates members on care gaps and assists with scheduling provider appointments
  • Links members to community resources addressing social determinants of health
Required Qualifications
  • High School Diploma / GED or equivalent work experience
  • Must be 18 years of age or older
  • 1+ years of call center and/or telephonic customer service experience
  • 1+ years of healthcare/insurance or social work/community outreach experience
  • Proficiency in Microsoft Word, Excel, and Outlook
