Job Summary
A company is looking for a Telephonic Care Navigator.
Key Responsibilities
- Completes telephonic outreach to DSNP/CSNP members using various outreach methods
- Educates members on care gaps and assists with scheduling provider appointments
- Links members to community resources addressing social determinants of health
Required Qualifications
- High School Diploma / GED or equivalent work experience
- Must be 18 years of age or older
- 1+ years of call center and/or telephonic customer service experience
- 1+ years of healthcare/insurance or social work/community outreach experience
- Proficiency in Microsoft Word, Excel, and Outlook
