Job Summary
Providing telephonic care and support, the full-time Telephonic Nurse Practitioner - CO Licensed will manage patient care coordination and deliver quality healthcare services remotely while fulfilling a commitment of 40 hours per week, including evenings, nights, and rotating weekends.
Key responsibilities:
- Deliver telephonic and virtual care to patients, coordinating with caregivers and healthcare facilities
- Conduct comprehensive assessments and document findings in compliance with regulations and quality standards
- Coordinate care transitions and adjust care plans based on patient needs and responses to treatment
Required qualifications:
- Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in Colorado
- Current Nurse Practitioner certification through a national board in a relevant specialty
- Proficient in using electronic medical records for documentation
- Ability to work scheduled shifts, including weekends and holidays
