Telephonic Nurse Practitioner - MA & NY Licensed

Job Summary

A company is looking for a Telephonic Nurse Practitioner with Massachusetts and New York licenses required.

Key Responsibilities:

• Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff
• Deliver quality care and manage both medical and behavioral conditions in collaboration with healthcare providers
• Coordinate care transitions and monitor the needs of members while adjusting care plans as necessary

Required Qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Massachusetts and New York
• Board certified as a Nurse Practitioner through a national board
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient computer skills for documenting medical information
