Job Summary
Providing telephonic and telehealth care, the full-time Telephonic Nurse Practitioner - MA & NY Licensed will manage patient care coordination, conduct assessments, and ensure quality care delivery while working remotely with a flexible schedule that includes evenings, nights, and rotating weekends.
Key responsibilities
- Coordinate and manage care for members through telephonic and video platforms, ensuring effective communication with caregivers and facilities
- Deliver comprehensive assessments and document findings in compliance with regulations while managing both medical and behavioral health conditions
- Facilitate care transitions and monitor patient needs, adjusting care plans as necessary to enhance outcomes and reduce costs
Required qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in Massachusetts and New York
- Board certification through a national board in Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nursing
- Current DEA licensure or ability to obtain post-hire, per state regulations
- Proficient computer skills for documenting medical information in electronic medical records
