Telephonic Nurse Practitioner - MA & NY Licensed

Job Summary

Providing telephonic care for patients, the full-time Telephonic Nurse Practitioner - MA & NY Licensed will manage care coordination and deliver quality healthcare remotely, requiring evening, night, and weekend shifts.

Key responsibilities:

• Provide telephonic and virtual care, coordinating with caregivers, facilities, and healthcare providers
• Conduct comprehensive assessments and document findings in compliance with regulations
• Coordinate care transitions and monitor patient needs, adjusting care plans as necessary

Required qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner licenses in Massachusetts and New York
• Active Nurse Practitioner certification through a national board in a relevant specialty
• Current DEA licensure or ability to obtain it post-hire, per state regulations
• Proficient computer skills for documenting medical information in electronic medical records
