Job Summary
Providing telephonic care and support, the full-time Telephonic Nurse Practitioner - MA & NY Licensed will manage patient care coordination and deliver quality virtual healthcare services while working remotely with a flexible schedule that includes evenings, nights, and rotating weekends.
Key responsibilities:
- Coordinate and manage care for members through telephonic and video platforms, ensuring effective communication with caregivers and healthcare providers
- Deliver comprehensive assessments and document findings in compliance with regulatory requirements while managing both medical and behavioral health conditions
- Facilitate care transitions and monitor patient needs, adjusting care plans as necessary to improve outcomes and reduce costs
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner licenses in Massachusetts and New York, with the ability to obtain additional licenses as needed
- Board certification through a national board in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner
- Current DEA licensure or ability to obtain it post-hire, per state regulations
- Proficient computer skills for documenting medical information in electronic medical records
