Telephonic Nurse Practitioner - New England

Job Summary

A company is looking for a Telephonic Nurse Practitioner with a New England License (CT, ME, NH, RI, NJ).

Key Responsibilities:

• Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff, ensuring 24/7 coverage
• Deliver cost-effective, quality care while managing medical and behavioral conditions in collaboration with other healthcare providers
• Coordinate care transitions and monitor patient needs, making adjustments to care plans as necessary

Required Qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Connecticut, Maine, New Hampshire, Rhode Island, or New Jersey
• Active Nurse Practitioner certification through a national board in a relevant specialty
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient computer skills for documenting medical information
