Job Summary

Providing telephonic care and coordination for patients, the full-time Telephonic Nurse Practitioner - New England will manage patient interactions and collaborate with healthcare teams remotely, ensuring quality care delivery across multiple states including New York, New Jersey, and Connecticut.

Key responsibilities:
  • Deliver telephonic and virtual care to patients, coordinating with caregivers and healthcare facilities
  • Perform comprehensive assessments and document findings in compliance with regulations
  • Coordinate care transitions and evaluate plans to ensure cost-effective and quality patient outcomes
Required qualifications:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted Nurse Practitioner license in relevant states
  • National board certification as a Nurse Practitioner in a specified specialty
  • Current DEA licensure or ability to obtain it post-hire
  • Proficient computer skills for documenting medical information
