Job Summary
Providing telephonic care for patients, the full-time Telephonic Nurse Practitioner - New England Licensed will manage care coordination, deliver quality medical services, and ensure compliance with documentation requirements while working remotely with flexible scheduling that includes evenings, nights, and rotating weekends.
Key responsibilities:
- Manage telephonic communication to coordinate care among patients, caregivers, and healthcare facilities
- Deliver acute care and perform comprehensive assessments while documenting findings in compliance with regulations
- Coordinate care transitions and monitor patient needs, facilitating adjustments to care plans as necessary
Required qualifications:
- Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states and ability to obtain in others as needed
- Active Nurse Practitioner certification through a national board in relevant specialties
- Current DEA licensure or ability to obtain post-hire, per state regulations
- Proficient computer skills for documenting medical information in electronic medical records
