Job Summary
Providing telephonic care for patients across multiple states, the full-time Telephonic Nurse Practitioner - New England Licensed will manage care coordination, perform assessments, and deliver quality healthcare remotely, requiring evening, night, and weekend shifts.
Key responsibilities
- Manage telephonic care and coordination for patients, caregivers, and facility staff, ensuring effective communication and documentation
- Deliver comprehensive assessments and manage acute and chronic conditions in collaboration with healthcare providers
- Coordinate care transitions and monitor patient needs to adjust care plans as necessary, ensuring quality and cost-effective care delivery
Required qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states (New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island)
- Current Nurse Practitioner certification through a national board
- Active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
- Proficient computer skills for documenting medical information in electronic medical records
