Job Summary
Providing telephonic and telehealth care, the full-time Telephonic Nurse Practitioner - New England Licensed will manage patient care coordination and deliver quality medical services remotely, requiring evening, night, and weekend shifts.
Key responsibilities:
- Coordinate and manage care for members through telephonic and video platforms, ensuring effective communication with caregivers and facilities
- Deliver comprehensive assessments and document findings in compliance with regulatory requirements while managing both acute and chronic conditions
- Facilitate care transitions and monitor patient needs, adjusting care plans as necessary to enhance patient outcomes and reduce costs
Required qualifications:
- Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states and ability to obtain in others as required
- Board certification through a national board as a Nurse Practitioner in relevant specialties
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
- Proficient computer skills for documenting medical information in electronic medical records
