Job Summary

Providing telephonic care and coordination for patients, the full-time Telephonic Nurse Practitioner - New England Licensed will manage medical and behavioral conditions remotely, ensuring quality care delivery and documentation while covering various shifts, including evenings, nights, and weekends.

Key responsibilities
  • Deliver telephonic and telehealth care, coordinating with caregivers, facilities, and providers to manage patient needs
  • Perform comprehensive assessments and document findings in compliance with healthcare regulations and quality standards
  • Coordinate care transitions and adjust care plans based on patient needs and treatment responses
Required qualifications
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted Nurse Practitioner license in New England states, with the ability to obtain additional licenses as needed
  • Board certification as a Nurse Practitioner through a national board
  • Current DEA licensure or ability to obtain post-hire, per state regulations
  • Proficient in electronic medical records and documentation practices
