Job Summary
Providing telephonic care for patients, the full-time Telephonic Nurse Practitioner - New England Licensed will manage virtual consultations and coordinate care across various settings, requiring evening, night, and weekend availability in a remote capacity.
Key responsibilities:
- Deliver telephonic and telehealth care while coordinating with caregivers, facilities, and providers
- Conduct comprehensive assessments and document findings in compliance with regulations
- Coordinate care transitions and monitor member needs to adjust care plans as necessary
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states and ability to obtain in additional required locations
- Current Nurse Practitioner certification through a national board
- Active DEA licensure or ability to obtain post-hire as per state regulations
- Proficient in using electronic medical records for documentation
