Job Summary
Providing telephonic care for patients across various settings, the full-time Telephonic Nurse Practitioner - New England Licensed will manage care coordination, conduct assessments, and deliver quality healthcare remotely while ensuring compliance with documentation requirements.
Key responsibilities
- Coordinate and manage care for members through telephonic and video platforms, collaborating with caregivers and healthcare providers
- Deliver acute care by performing comprehensive assessments and documenting findings in compliance with regulations
- Facilitate care transitions and monitor member needs, adjusting care plans as necessary to ensure effective treatment
Required qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states, with the ability to obtain licenses in additional required locations
- Current Nurse Practitioner certification through a national board in Family, Adult, Gerontology, or Adult-Gerontology Acute Care
- Current DEA licensure or ability to obtain post-hire, per state regulations
- Proficient computer skills for documenting medical information in electronic medical records
