Job Summary
Providing telephonic care, the part-time Telephonic Nurse Practitioner - New England Licensed will manage patient assessments and care coordination remotely, requiring evening, night, and weekend shifts while ensuring compliance with healthcare regulations.
Key responsibilities:
- Deliver telephonic and virtual care to patients, coordinating with caregivers and healthcare facilities
- Perform comprehensive assessments and document findings in compliance with CMS regulations
- Coordinate care transitions and monitor patient needs, adjusting care plans as necessary
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states (New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island)
- Board certification as a Nurse Practitioner through a recognized national board
- Current DEA licensure or ability to obtain post-hire, per state regulations
- Proficient in electronic medical records and documentation practices
