Telephonic Nurse Practitioner - OH MO WI IN

Job Summary

A company is looking for a Telephonic Nurse Practitioner (Part-Time) with an active license in OH, MO, WI, or IN.

Key Responsibilities:

• Provide telephonic and telehealth care to patients, caregivers, and facility staff, ensuring 24/7 coverage
• Conduct comprehensive assessments, document findings, and manage both medical and behavioral conditions
• Coordinate care transitions and monitor patient needs while facilitating adjustments to care plans

Required Qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted NP license in the state of residence and in OH, MO, IN, or WI
• Board certified as a Nurse Practitioner in relevant specialties
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient computer skills for documenting medical information
