Job Summary

Providing telephonic care and support, the full-time Telephonic Nurse Practitioner - OH, MO, WI, IN will manage patient care coordination and deliver quality virtual healthcare services remotely, requiring evening, night, and weekend shifts.

Key responsibilities:

• Coordinate and manage care for members through telephonic and video platforms, ensuring patient privacy and compliance
• Deliver comprehensive assessments and document findings in accordance with CMS regulations while managing both medical and behavioral health conditions
• Facilitate care transitions and monitor patient needs, adjusting care plans as necessary to optimize outcomes and reduce costs

Required qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Ohio, Missouri, Indiana, or Wisconsin
• National board certification as a Nurse Practitioner in one of the specified specialties
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient in electronic medical records and documentation practices
