Job Summary

A company is looking for a Telephonic Nurse Practitioner with an OH, MO, WI, or IN License required.

Key Responsibilities:

• Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff, ensuring 24/7 coverage
• Deliver cost-effective, quality care while managing medical, behavioral, chronic, and acute conditions
• Coordinate care transitions and monitor member needs, adjusting care plans as necessary

Required Qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted license in the state of residence and in Ohio, Missouri, Indiana, or Wisconsin
• Active Nurse Practitioner certification through a national board in relevant specialties
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient computer skills for documenting medical information
