Job Summary

A company is looking for a Telephonic Nurse Practitioner with an OH, MO, WI, or IN License Required.

Key Responsibilities:

• Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff
• Deliver cost-effective, quality care while managing medical and behavioral conditions
• Coordinate care transitions and monitor member needs while facilitating adjustments to care plans

Required Qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in the state of residence and additional required states
• Board certified through a national board in relevant nurse practitioner specialties
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient computer skills for documenting medical information using electronic records
