Job Summary

A company is looking for a Telephonic Nurse Practitioner with an OH, MO, WI, or IN License Required.

Key Responsibilities:

• Provide telephonic and telehealth care to patients, caregivers, and facility staff, ensuring 24/7 coverage
• Deliver quality care by managing medical, behavioral, chronic, and acute conditions in collaboration with healthcare providers
• Coordinate care transitions and evaluate care plans to ensure cost-effectiveness and alignment with patient needs

Required Qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in the state of residence and OH, MO, WI, or IN
• Active Nurse Practitioner certification from a national board in Family, Adult, Gerontology, or Adult-Gerontology Acute Care
• Proficient computer skills for documenting medical information in electronic medical records
• Ability to work scheduled shifts in accordance with scheduling policies
