Job Summary

Providing telephonic care for patients, the full-time Telephonic Nurse Practitioner - OH, MO, WI, IN Licensed will manage virtual consultations, coordinate care, and deliver quality healthcare services remotely, requiring evening, night, and weekend shifts.

Key responsibilities:

• Deliver telehealth services by coordinating care between patients, caregivers, and healthcare facilities
• Perform comprehensive assessments and document findings in compliance with healthcare regulations
• Coordinate care transitions and monitor patient needs to ensure effective treatment plans

Required qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Ohio, Missouri, Indiana, or Wisconsin
• Active Nurse Practitioner certification through a national board
• Current DEA licensure or ability to obtain it post-hire as per state regulations
• Proficient in using electronic medical records for documentation
