Job Summary

Providing telephonic care and support, the part-time Telephonic Nurse Practitioner - OH, MO, WI, IN Licensed will manage patient care coordination, deliver quality medical services, and document clinical findings while working remotely with flexible scheduling that includes evenings, nights, and rotating weekends.

Key responsibilities

• Coordinate and manage care for members through telephonic communication with caregivers, facilities, and healthcare providers
• Deliver cost-effective, quality care by managing both medical and behavioral conditions in collaboration with physicians
• Document patient assessments and ensure compliance with regulatory requirements while facilitating care transitions

Required qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Ohio, Missouri, Indiana, or Wisconsin
• Board certification through a national board as a Nurse Practitioner in relevant specialties
• Current DEA licensure or ability to obtain post-hire as per state regulations
• Proficient computer skills for documenting medical information with electronic medical records
