Job Summary

Providing telephonic care and support, the full-time Telephonic Nurse Practitioner - OH, MO, WI, or IN Licensed will manage patient care coordination and deliver quality virtual healthcare services remotely, with evening, night, and weekend shifts required.

Key responsibilities:

• Deliver telephonic and virtual care, coordinating with patients, caregivers, and healthcare facilities
• Conduct comprehensive assessments and document findings in compliance with healthcare regulations
• Coordinate care transitions and monitor patient needs to ensure effective treatment plans

Required qualifications:

• Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Ohio, Missouri, Indiana, or Wisconsin
• Active Nurse Practitioner certification through a national board in a relevant specialty
• Current DEA licensure or ability to obtain it post-hire, per state regulations
• Proficient computer skills for documenting medical information in electronic medical records
