Telephonic Outreach Specialist

Job Summary

Working remotely in a full-time capacity, the Telephonic Outreach Specialist will engage with clients to provide support, facilitate access to services, and ensure a positive experience throughout their care journey.

Key responsibilities:

• Conduct telephonic outreach to clients to assess needs and provide information about available services
• Collaborate with healthcare teams to coordinate care and follow up on client progress
• Document interactions and maintain accurate records in the client management system

Required qualifications:

• Experience in a healthcare or outreach role, preferably involving telephonic communication
• Strong understanding of healthcare services and community resources
• Proficiency in using client management software and telecommunication tools
• Ability to work independently and manage time effectively in a remote environment
• High school diploma or equivalent; additional education in healthcare or social services is a plus
