Telephonic Outreach Specialist

Job Summary

Engaging with eligible members remotely, the full-time Telephonic Outreach Specialist will initiate the intake process, provide support, and connect members with their care teams while leveraging data to inform outreach efforts.

Key responsibilities

• Provides telephonic and virtual outreach to engage eligible members
• Initiates the intake process and facilitates handoffs to care teams
• Maintains accurate member information and communicates urgent needs to care teams

Required qualifications

• Minimum of 1 year of experience in a similar outreach role
• Familiarity with data management systems, preferably Commons
• Experience in collecting consent and managing demographic information
• Ability to work collaboratively with care teams and support members effectively
• Proven track record of engaging with diverse populations
