Telephonic Outreach Specialist
Job Summary
Connecting with eligible members remotely, the full-time Telephonic Outreach Specialist will engage in initial conversations to begin the intake process and serve as a point of contact until members are connected with a Community Health Partner.
Key responsibilities
- Provides telephonic and virtual outreach and engagement services to eligible members
- Initiates the intake process and facilitates handoffs to the care team while communicating urgent needs
- Collects and documents member information and consent, leveraging data to inform outreach processes
Required qualifications
- Minimum of 1 year experience in a similar role
- Familiarity with data management systems, preferably Commons
- Experience in telephonic outreach or member engagement
- Ability to work collaboratively with care teams and support staff
- Proficiency in documenting and organizing member information accurately
