Telephonic Outreach Specialist

Job Summary

Connecting virtually with eligible members, the full-time Telephonic Outreach Specialist will engage in initial conversations to assess needs and initiate the intake process, serving as the primary contact until a Community Health Partner is assigned, all while working remotely.

Key responsibilities:

• Provides telephonic and virtual outreach and engagement services to eligible members
• Initiates the intake process and facilitates handoffs to the care team while documenting necessary information
• Communicates urgent needs and relevant information to care teams and tracks performance within the Commons platform

Required qualifications:

• Minimum of 1 year of experience in a similar role
• Familiarity with telephonic outreach and engagement processes
• Experience using data management systems, specifically Commons or similar platforms
• Ability to collect verbal and written consent from members
• Understanding of community health services and care coordination
