Job Summary

A company is looking for a Telephonic Case Manager RN.

Key Responsibilities:
  • Serve as primary care manager for members identified for CVS Accountable Care
  • Telephonically engage members to complete a holistic assessment of current health status and work collaboratively with the care team
  • Develop and implement a care plan that addresses physical, behavioral, and social needs

Required Qualifications:
  • 3+ years clinical RN experience
  • 1+ year of experience in critical care, ICU, CCU, or Emergency
  • Unrestricted RN license in state of residence with multi-state/compact privileges
  • Managed Care experience preferred
  • Associate degree in nursing RN
