Job Summary
A company is looking for a Telephonic Case Manager RN.
Key Responsibilities:
- Serve as primary care manager for members identified for CVS Accountable Care
- Telephonically engage members to complete a holistic assessment of current health status and work collaboratively with the care team
- Develop and implement a care plan that addresses physical, behavioral, and social needs
Required Qualifications:
- 3+ years clinical RN experience
- 1+ year of experience in critical care, ICU, CCU, or Emergency
- Unrestricted RN license in state of residence with multi-state/compact privileges
- Managed Care experience preferred
- Associate degree in nursing RN
