Telephonic Social Worker Care Manager

Job Summary

A company is looking for a Telephonic Social Worker, Care Manager for remote positions in multiple states.

Key Responsibilities:

• Identify and resolve barriers that hinder effective care
• Ensure patient is progressing towards desired outcomes by continuously monitoring patient care
• Create member care plans and assess members' social needs

Required Qualifications:

• Master's degree in Social Work
• Current valid social work license
• Minimum 2 years of experience in identifying and accessing resources to meet basic needs
• Must possess effective telephonic and virtual communication skills
• Proficiency with Microsoft Office Programs
