A for-profit American managed health care company that markets and administers health insurance is searching for a person to fill their position for a Remote Home Visit Field Care Manager in Chase City-Danville.
Core Responsibilities Include:
- Evaluating member needs
- Implementing appropriate interventions with participants in their place of residence
- Completing physical, psychological, emotional and environmental reviews of your assigned members in order to provide appropriate, timely interventions to optimize care
Qualifications Include:
- MSW OR Registered Nurse with two years of experience of in home case/care management
- Experience working with the adult population
- Knowledge of community health and social service agencies and additional community resources
- Exceptional communication and interpersonal skills with the ability to quickly build rapport
