Job Summary

A physician-owned, physician-operated group of health plans and related companies is in need of a Teleworking Behavioral Health Community Care Coordinator in Chicago.

Must be able to:

• Manage behavioral health case team assignments
• Act as an advocate for the member, assuring they receive the supports and services they desire and are eligible to receive
• Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure member safety

Qualifications for this position include:

• Masters degree in Social Work, psychology, or counseling
• 1 years experience in lead, supervisory or management
• 2 years experience in discharge planning, home health care, community health or managed care
• Current clinical license (LCPC or LCSW) to practice in Illinois
