Tennessee Bilingual Care Coordinator

Job Summary

A company is looking for a Bilingual Care Coordinator to provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan members.

Key Responsibilities

• Conduct outreach to educate members about program benefits and facilitate their engagement
• Assess member status and develop personalized care plans addressing their goals
• Manage a caseload, ensuring timely communication and coordination of care with providers

Required Qualifications

• 2+ years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker
• Fluency in English and Spanish
• Must be located in Tennessee with a dedicated home office for remote work
• Experience working with Medicaid and/or Dual Eligible patients preferred
• LPN or RN preferred, but not required
