Key Responsibilities

Conduct outreach to educate members about program benefits and motivate participation
Assess member status and develop personalized care plans addressing their goals
Coordinate care with providers and manage a caseload to ensure timely member contact

Required Qualifications

Fluency in English and Spanish
2+ years of experience in care coordination or related fields
Experience working with Medicaid and/or Dual Eligible patients
Remote work experience preferred
Dedicated home office for remote work
