Job Summary

Tennessee Licensed Care Concierge, this is a full-time remote position responsible for supporting older adults with chronic conditions through care coordination, education, and advocacy.

Key Responsibilities

• Serve as the primary contact for patients, building relationships and conducting regular health check-ins
• Navigate patients through the healthcare system and coordinate care with providers and community resources
• Conduct screenings for social determinants of health and connect patients to appropriate resources

Required Qualifications and Education

• Authorized to work in the US without employee sponsorship
• Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential
• 1+ years of patient-facing healthcare experience in relevant settings
• High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred
• Proficient with EHR systems and capable of managing a caseload independently
