Job Summary
Tennessee Licensed Care Concierge, this is a full-time remote position responsible for supporting older adults with chronic conditions through care coordination, education, and advocacy.
Key Responsibilities
- Serve as the primary contact for patients, building relationships and conducting regular health check-ins
- Navigate patients through the healthcare system and coordinate care with providers and community resources
- Conduct screenings for social determinants of health and connect patients to appropriate resources
Required Qualifications and Education
- Authorized to work in the US without employee sponsorship
- Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential
- 1+ years of patient-facing healthcare experience in relevant settings
- High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred
- Proficient with EHR systems and capable of managing a caseload independently
