Job Summary
To support a growing practice, the full-time Tennessee Licensed Care Coordinator will provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible members, building trusted relationships and addressing both medical and social needs in a remote setting.
Key Responsibilities
- Conduct outreach to educate members about program benefits and assess their status to develop personalized care plans
- Manage a caseload of members, facilitating coordination of care with providers and ensuring timely communication of medical information
- Document actions taken in a care management platform and identify needs related to Social Determinants of Health
Required Qualifications
- Active and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
- Fluency in both Spanish and English
- 2+ years of experience in care coordination, case management, or related fields
- 1+ years of remote work experience
- Experience working with Medicaid and/or Dual Eligible patients
