Tennessee Licensed Care Coordinator

Job Summary

To support a growing practice, the full-time Tennessee Licensed Care Coordinator will provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible members, building trusted relationships and addressing both medical and social needs in a remote setting.

Key Responsibilities

• Conduct outreach to educate members about program benefits and assess their status to develop personalized care plans
• Manage a caseload of members, facilitating coordination of care with providers and ensuring timely communication of medical information
• Document actions taken in a care management platform and identify needs related to Social Determinants of Health

Required Qualifications

• Active and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
• Fluency in both Spanish and English
• 2+ years of experience in care coordination, case management, or related fields
• 1+ years of remote work experience
• Experience working with Medicaid and/or Dual Eligible patients
