Job Summary

A company is looking for a Care Coordinator to provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan members.

Key Responsibilities

• Conduct outreach to educate members about program benefits and facilitate care coordination
• Assess member status and develop care plans to address individual goals and needs
• Manage a caseload, ensuring timely communication and coordination with healthcare providers

Required Qualifications

• 2+ years of experience in care coordination, case management, community health work, or related fields
• Experience working with Medicaid and/or Dual Eligible patients
• Remote work experience preferred
• Experience documenting case notes in care management or electronic health record platforms
• Dedicated home office for remote work
