Job Summary
As a Family Preservation Services Care Coordinator, the full-time position based in Cookeville, Tennessee, will manage client cases, provide intervention, and serve as a resource link for children and families in need of assistance.
Key responsibilities
- Manage client cases and provide intervention for children and families
- Collaborate with treatment teams to develop and evaluate care plans
- Advocate for individuals by connecting them to organizations that improve their situations
Required qualifications
- Bachelor's degree in a Human Service discipline from an accredited institution
- CPR and First Aid certified, or willingness to obtain certification
- Experience working with children/adolescents in a therapeutic, community-based environment
- Ability to work flexible hours and at multiple locations as needed
